1098-T forms are provided for currently enrolled students and are available no later than January 31st every year. Please note that only students enrolled in curriculum classes receive a 1098-T form.
1098-T forms can be viewed and printed as soon as they are available through Self-Service, eliminating the need to wait for them in the mail. Taking advantage of this option is easy:
- Log into your account and click Tax Information > 1098 Information > and give your consent.
After giving your consent, you will no longer receive a copy in the mail.
IMPORTANT HEALTH COVERAGE TAX DOCUMENT (1095-C FORM)
According to new Federal requirements, you may receive a copy of your Form 1095-C upon request. This means that 91ÇÑ×Ó is no longer required to automatically send you a 1095-C.
If you would like a copy of your Form 1095-C, please access our benefits portal at and download your Form 1095-C electronically.
Alternatively, if you need us to mail your Form 1095-C, please contact us as noted below. We will send your form no later than 30 days after we receive your request.
If you are no longer employed by 91ÇÑ×Ó or you retired in 2025, you will receive your Form 1095-C in the mail.
